A&D Cares provides a professional Care Management team for fiduciary clients in need of in-home assistance and/or monitoring in care facilities. A&D Cares has staff that is professional, friendly and caring. Our team establishes realistic short and long range care goals, and coordinates with available community services.
When caring for a loved one, a professional fiduciary can assist in lessening the strains on the family for the time commitment required for providing quality care. Our services respect the dignity and uniqueness of each client. Through a well thought out, interdisciplinary approach, A&D Cares assists people who need personal, readily accessible services.
Our support services also include money management, assessments and evaluations. We are always available to consult with you and your family to discuss long term care needs and solutions.

- Home care needs assessment
- Facilities assessment and referrals
- In home or institutional care evaluation and monitoring
- Physician and nursing staff coordination and advocation
- Determination and/or coordination of placement for all levels of care
- Supplemental, social, medical and companion services arrangements, including adult day care and other personal care services (through accredited agencies)
- Agent under Health Care Directives (available only to those persons for whom we are acting as trustees or successor trustees)
